Dear Prospective Applicant,
Thank you for your interest in Northwell Health’s ACP Fellowship Programs.
Our ACP Surgical Fellowship at Norwalk Hospital is a 12-month postgraduate certificate program for NCCPA eligible/certified physician assistants who will hold at least a bachelor’s degree by the start of the residency year. To complete your application, please complete the following steps by March 15:
Step 1: Complete your online application form: Applications are currently closed.
Step 2: Email the following to surpares@nuvancehealth.org.
- Your current resume
- A one-page, typewritten narrative stating why you are interested in becoming a Surgical PA
- Three current signed professional letters of recommendation sent directly from your references – one must be from your PA Program Director, Academic or Clinical Coordinator. Your signed letters need to be emailed directly to surpares@nuvancehealth.org. Please have them call/email us directly, if they cannot send by email.
- Official copy of NCCPA verification letter (if certified)
- All college and PA program transcripts – unofficial copies must be sent electronically to surpares@nuvancehealth.org. We only accept electronic transcripts. We reserve the right to request official transcripts at any time during the application process.
- If schools don’t have e-transcript services, please mail the transcript to the Admission Committee address below – this address is only for non-electronic transcripts and checks, please do not mail any other documents.

Our application deadline is March 15 each year for September entry to the program. As application files are completed, they will be reviewed and selection will be made for a formal interview.
