Application Requirements:

• Applicant must be enrolled in or is a graduate of an ACPE-accredited school of pharmacy
• Applicant must be eligible for Connecticut pharmacist licensure by the start of the residency
  year
• Applicant must submit to PhORCAS by the specified deadline the following:
  • Letter of Intent – including description of personal and professional goals as they relate
    to clinical pharmacy practice
  • Curriculum vitae
  • Official School of Pharmacy transcript
  • Three letters of recommendation with at least one from a clinical rotation preceptor
    Qualified applicants will be invited to complete a virtual interview; elective onsite tours will be available.
